Advanced Practice Provider (Primary Care)

Join Together Health as a founding Advanced Practice Provider in the Seattle area. We are an innovative, in-home and virtual primary care practice built around a subscription-based model that gives you time to truly care for patients.

In this role you will deliver holistic primary care via telehealth, in-person, and in-home visits in and around Seattle, WA. You will collaborate with our clinical team, develop individualized care plans, coordinate care, and help shape protocols for a growing regional presence.

You may live anywhere in the greater Seattle metro area, but an active, unrestricted Washington NP or PA license is required.

What you will do

• Provide comprehensive primary care to adult patients through telehealth, in-office, and in-home visits
• Perform assessments, diagnose and treat acute and chronic conditions, and order and interpret diagnostics
• Develop and manage individualized, whole-person care plans with an emphasis on prevention
• Prescribe and manage medications, including chronic disease management
• Coordinate care with specialists, community resources, and the Together Health care team
• Document accurately and timely in the EMR
• Help build and refine clinical protocols as we grow our regional presence

Requirements

• Active, unrestricted Washington Nurse Practitioner or Physician Assistant license (or ability to obtain before start)
• Board certification as an NP (FNP/AGNP preferred) or PA
• Active, unrestricted DEA registration (or eligibility to obtain)
• Master's degree or higher from an accredited NP or PA program
• Minimum 2 years of patient-facing clinical experience; primary care experience strongly preferred
• Valid, unrestricted driver's license, reliable transportation, and willingness to travel for in-home visits
• Authorized to work in the United States
• Comfortable with telehealth platforms and electronic medical records
• No history of malpractice claims or license restrictions

Preferred qualifications

• Experience in home-based, house-call, or direct primary care settings
• Experience with value-based or subscription-based care models
